Don t miss the opportunity to be a part of our Inpatient Rehab Unit within Hartford Hospital. All 26 beds are in private inpatient rooms giving our patients access to ongoing acute medical management, 24 hour rehabilitative nursing care and the latest rehab techniques. Our dedication to delivering personalized coordinated care coupled with the healing environment offered by this new unit will shorten length of stay and allow our patients a quicker and healthier return to home.

Overview
We re seeking dedicated and caring Case Coordinators to be part of a team with a patient-centered approach, bringing decades of experience in providing the most innovative care. This position will assess patients with all diagnoses with a focus on stroke, traumatic brain or spinal cord injuries, as well as other neurological conditions. Case Coordinators will coordinate patient care and services across the care continuum, while identifying risk factors as well as develop and implement transition planning activities. Our new unit features a collaborative interdisciplinary team atmosphere and approach to patient care. This is a great opportunity to work in a special inpatient rehab setting, while still having access to the opportunities and benefits provided by an acute care hospital.

Qualifications

Requirements
Current state licensure or eligibility for state licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT) or Speech Therapist (ST) required.
Three years diversified Case Management, home care, and/or managed care experience
Inpatient rehabilitation experience preferred.
Certification in Case Management strongly preferred
